REPORT DISCIPLINARY ISSUES#
The Report Disciplinary Issues (RLDI) form allows you to produce a report providing information about the disciplinary issues created in ILDI.Report Parameters & Filters#

| Show Activities | This field allows you to specify whether they would like the activities associated with the disciplinary issue to be included in the report |
| Show Occurrences | This field allows you to specify whether they wish the occurrences associated with the incident to be included in the report. |
| From Date | If this report is to cover a specific time frame, this field allows you to specify the first date of that time frame. |
| To Date | If this report is to cover a specific time frame, this field allows you to specify the last date of that time frame. |
| User Comment | This field holds the user-supplied comment which will appear in the header of all pages of the report. |
