REPORT INJURIES AND ILLNESSES (OSHA 300, 300A, 301)#

The Report Injuries and Illnesses (RHOSW) will provide you with a detailed report of injuries and illnesses in the OSHA formats.

Excel report includes OSHA Form 300, OSHA Form 300A, OSHA Form 301. OSHA Form 301 is for each individual. RHOSW will generate Form 301 in new tabs for each individual.

Must have 'DAILY CALENDAR' in IDCL. It is being used to derive annual average number of employees.

"Annual average number of employees" in 300A is calculated by counting the number of distinct Identities that have been hired and not terminated each month of the year, summing those up and dividing by 12 (rounded).

Report Parameters & Filters#

Report Parameters
EntityThis field allows you to define the entity for which you wish the report published.
LocationThis field allows you to define the location for which you wish the report published
Calendar YearThis field allows you to define the calendar year for which you wish the report published.
Hrs Worked Pay ElementUsed on 300A to calculate "Total hours worked by all employees last year"
Company ExecutiveUsed on 300A in the "Sign here" box
Title Used on 300A in the "Sign here" box
Phone Used on 300A in the "Sign here" box
Completed by Name of person who completed the incident report on 301 in "Completed by" box
Title (Completed by) Title of person who completed the incident report on 301 in "Completed by" box
Phone (Completed by) Phone of person who completed the incident report on 301 in "Completed by" box

Report Filters
Case NumberThis field allows you to indicate which case numbers should appear on the report.
IncidentThis field allows you to indicate which incidents should appear on the report.
