MY T4 CONSENT#

The My T4 Consent (WEPT4) screen allows you to choose to receive Electronic T4 forms or to withdraw your consent to receive the electronic forms.

The definition data for the My T4 Consent screen is stored in the P2K_HR_EMPLOYMENTS table.

The fields listed on the form are as follows:

Employee

The employee’s Last name and First name is displayed here.

Consent Status

The status for the Electronic printing, either Consent Received or Withdrawn

CLICK HERE TO INDICATE CONSENT TO RECEIVE ELECTRONIC T-4 FORMS

This button will allow you to choose to receive the Electronic T4 forms. If this button is pressed, then the Consent Status will be changed to ‘Consent Received’.

CLICK HERE TO WITHDRAW CONSENT TO RECEIVE ELECTRONIC T-4 FORMS

If the employee has already consented to receive electronic T4 forms, they may use this button to withdraw their consent. If this button is pressed, then the Consent Status will be changed to ‘Withdrawn’.
