MY T4 CONSENT#

The My T4 Consent (WEPT4) form allows employees to choose to receive Electronic T4 forms or to withdraw their consent to receive the electronic forms.

The definition data for the My T4 Consent screen is stored in the P2K_HR_EMPLOYMENTS table.

WEPT4 Form Fields

Field	Description
Employee	The employee’s Last name and First name is displayed here.
Consent Status	GR_ELECTRONIC_CONSENT:The status for the Electronic printing. Options are: Consent Received, Withdrawn or No Consent Received
Click here to indicate consent to receive electronic T-4 forms	GR_ELECTRONIC_CONSENT This button confirms consent to receive the Electronic T4 forms. When this button is pressed, the Consent Status will be changed to ‘Consent Received’.
Click here to withdraw consent to receive electronic T-4 forms	GR_ELECTRONIC_CONSENT If an employee has already consented to receive electronic T4 forms, pressing this button will withdraw their consent. When this button is pressed, the Consent Status will be changed to ‘Withdrawn’.
