My Dependents #

The My Dependents screen will allow you to edit information about individuals you have listed as your dependents in the My Contacts screen. You may not add or delete a dependent, however, you may update their contact information such as address or phone numbers.

At the top of the screen, you will see a list of dependents listed by:

• First Name
• Last Name
• Relation
• Phone #
• Phone Ext.

By clicking on one of the dependents listed, you will be able to see further details about them in the following fields.

Dependent Details#

First Name

This is the first name used by the dependent. (Mandatory)

Last Name

This is the surname of the dependent. (Mandatory)

Date of Birth

This field displays the dependent’s date of birth (used for tax and benefit purposes).

Gender

This field displays whether the dependent is male or female.

Address 1

This is the home address (street or rural route or concession #) of the dependent.

Address 2

This field displays the second line of address, used for PO Box # or special delivery instructions.

City

This field refers to the city, district or township name where the dependent resides.

State, Country

This field displays the dependent’s state and country.

Zip Code

This field displays the zip or postal code for your dependent’s address.

Phone #

This field displays the phone number of your dependent.

Phone Ext.

This field displays the phone extension of your dependent; if applicable.

Relation

This field describes the relationship of the dependent to you.

Government Code

The dependent’s Social Security Number/Social Insurance Number is displayed in this field.

e.g. 648467354 will be formatted to 648-46-7354 for the US

648467354 will be formatted to 648-467-354 for Canada

Phone # / Extn

These field provide the phone number and extension for the dependent.

Alt Phone #

This is the alternative phone number for the dependent; if applicable.

Alt Ext

This field displays the phone extension of you dependent’s alternative phone number.

Cellular #

This is the cellular, portable or mobile phone number for the dependent.

E-mail

This is the email address of the dependent.

Plans that this Dependent/Beneficiary is registered with

Plan

This field identifies the plan the dependant/beneficiary are registered with.

Desc

This field gives a short description of the plan.

Start / End Date

This field defines a time frame for the registration.

Benefit %

This field indicates the percentage of the benefit to which the dependent/beneficiary.
