MY DEPENDENTS#

The My Dependents (WEBDP) screen allows you to edit information about individuals you have listed as your dependents in the My Contacts screen. The definition data for the My Dependents screen is stored in the P2K_HR_CONTACTS and P2K_BE_BENEFIT_RECIPIENTS tables.

At the top of the screen, your dependants are listed by the following fields:
First Name
This is the first name used by the dependent. (Mandatory)
Last Name
This is the surname of the dependent. (Mandatory)
Middle Name
This is the middle name of the dependent.
Phone #
This field displays the phone number of your dependent.
Phone Ext.
This field displays the phone extension of your dependent; if applicable.
Status:This field displays what status the dependent is (Active/Inactive).
Beneficiary
If the dependent was defined with a Contact Type of Beneficiary Contact, 'Yes' will display in this field.
Dependent
If the dependent was defined with a Contact Type of Dependent, 'Yes' will display in this field.
Emergency
If the dependent was defined with a Contact Type of Emergency Contact, 'Yes' will display in this field.

By clicking on one of the dependent listed, you will be able to see further details about that dependent in the following fields.

Dependent Details#

Salutation
This field field displays the title of the dependent.
First Name
This field displays the first name of your dependent. (Mandatory)
Last Name
This field displays the surname of your dependent. (Mandatory)
Middle Name
This is the middle name of the dependent.
Gender
This field displays your dependent’s gender.
Relation
This field displays the relationship between you and your dependent.
Rank
S.I.N
The dependent’s Social Security Number/Social Insurance Number is displayed in this field.
Date of Birth
This is the date of birth of your dependent (used for tax and benefit purposes).
Age
This field shows the dependent’s age in years.
Address 1
This field displays the dependent’s address (street or rural route or concession #).
Address 2
This field displays the second line of dependent’s address. This field may display the PO Box # or special delivery instructions.
City
This field displays the city, district or township for your dependent’s address.
State, Country
This displays the state and country of your dependent’s address.
Postal Code
This field displays the zip or postal code for your dependent’s address.
Phone #
This field displays your dependent’s main phone number.
Ext.
This field displays the phone extension of your dependent, if applicable.
Alt Phone #
This field displays the cellular phone number for your dependent.
Email
This is the dependent’s email address.
Cellular #
This is the cellular, portable or mobile phone number for the dependent.
Disabled Indicator
This toggle indicates the dependent is Disabled. This toggle is used by the Open Enrollment logic in Benefit Self Service.
Student Indicator
This toggle is used to indicate if the dependent is a student. This toggle is used by logic for the Open Enrollment process in Benefit Self Service.
Student Until
This date field is used to indicate the last date the dependent is considered a student. This field is used for the Open Enrollment process in Benefit Self Service.
Beneficiary
This toggle indicates the dependent is a Beneficiary. This toggle is used by the Open Enrollment logic in Benefit Self Service.
Dependent
This mandatory toggle. This toggle is used by the Open Enrollment logic in Benefit Self Service.
Emergency
This toggle indicates the dependent is an Emergency contact. This toggle is used by the Open Enrollment logic in Benefit Self Service.
Spouse
This toggle indicates the dependent is a Spouse. This toggle is used by the Open Enrollment logic in Benefit Self Service.

Plans that this Dependent/Beneficiary is registered with#

Plan
This field identifies the plan the dependent/beneficiary are registered with.
Description
This field gives a short description of the plan.
Start / End Date
Theese fields define a time frame for the registration.
Benefit %
This field indicates the percentage of dependent coverage.
