As of Date	Mandatory, Date, LOV available

Print Details	Optional, Yes or No, LOV available If <Yes> then the policy details (components, rules, etc.) will be displayed on the report, otherwise no details will display on the report.
User Comment	Optional, Text This field holds the user-supplied comment which will appear in the header of all pages of the report.

Work Rule	Optional, LOV available Limits the process to the work rule specified.
